• Please bring any pertinent information including dental insurance card, current x-rays and current physicians information.
• Please arrive 10 – 15 minutes early in order to complete the necessary paperwork (this is also available at www.foydentalcare.com).
• We will perform the following

1. Any necessary x-rays – typically a full mouth series (18 x-rays) on adults or a Panorex and bitewings on a child.
2. A detailed exam of the soft tissues of the mouth.
3. A detailed exam of the teeth.
4. A periodontal exam (gums)

• From this information we will form a comprehensive treatment plan and place the items in order of importance. Our front office will then present the financial aspect of the treatment including any insurance that may be involved.
